How do I fill out the Account Section (Section A)?
DW Account Number: This can be found on the top left of your DriveWealth monthly account statement. If you have trouble locating your account number, please view this article.
Account Holder Name: Provide your name as it appears on your DriveWealth account.
Phone Number: Provide your phone number as it appears on your DriveWealth account.
Email Address: List the email address as it appears on your DriveWealth account.
How do I fill out the Instructions Section (Section B)?
Company Name: The name of the stock being transferred.
Ticker: The stock symbol (e.g. Apple is AAPL).
CUSIP: Stock identification number.
Number of Whole Shares: Only whole shares only can be transferred. Fractional shares cannot be transferred and a request for fractional shares will result in rejection of your transfer request.
How do I fill out the Receiving Firm’s/Transfer Agent Details Section (Section C)?
Receiving Firm Name: Receiving firm name (e.g. Computershare).
Receiving Firm DTC Number: Contact the receiving firm directly for this information.
Receiving Firm Address: Enter the receiving firm mailing address.
Shares should be registered for the benefit of: Please write your name as it appears on your DriveWealth account.
Receiving Firm Account Number if Applicable: Enter existing receiving firm account number.
How do I fill out the SSN/Address Information Section(Section D)?
SSN/Tax # (US); Add 999-99-999 (Non-US): If you do not have a U.S. Social Security Number (SSN), write 999-99-9999 in this field.
Address Line1: Provide the information from address line 1 on your DriveWealth account.
Address Line 2: Provide the information from address line 2 on your DriveWealth account (if any).
City and State: Provide the information from the City and State fields on your DriveWealth account.
Postal/Zip Code: Provide the information from the Postal/Zip code on your DriveWealth account.
Country: Provide your Country of residence as listed on your DriveWealth account.
IMPORTANT: The line items on the form must exactly match the address on your DriveWealth account or your request will be rejected. You can view the address on your account here.

Best Practices for filling out your DRS Form
- Sign and Date the form.
- Submit the form in 1 pdf. We are unable to process forms that come in 2 or more attachments.
- If you are providing a Receiving Firm Account number, please verify your account is able to accept the positions you wish to transfer. This can cause transfer from being rejected/delayed.
- Save and submit 1 DRS for all the shares you wish to transfer. Multiple transfers without a Receiving Firm Account Number can create duplicate accounts.
